A Neglected Posterior Dislocation of the Sternoclavicular Joint

Abstract
The posterior dislocation of a sternoclavicular joint is very uncommon and is easily neglected because it is usually combined with other serious injuries and discloses no specific symptoms or signs of dislocation. Moreover the routine radiologic examinations are very hard to confirm the diagnosis.
The posterior dislocation of the clavicle at sternoclavicular joint often injure the vital structures such as trachea, esophagus and great vessels which are located posterior to this joint. The surgical treatment of this problem is risky procedure and serious post-operative complications are known more common to occur.
Authors found a neglected posterior dislocation of the sternoclavicular joint in a 32 years old woman who had multiple fractures by an automobile accident 8 weeks previously. The diagnosis was confirmed by computerized tomogram. She had no serious combined injury directly related to this dislocation. An open reduction was tried at 9 weeks after injury and the reduced position was fixed by transfixation of 2 Kirschner wires and stabilized the medial end of clavicle to the first rib using fascia lata.
The post-operative course was uneventful and the follow-up examination performed at 8 months after surgery revealed stable reduced joint and full range of motion of the shoulder was regained.
